1.3編制依據(jù) 《中華人民共和國突發(fā)事件應對法》、《中華人民共和國傳染病防治法》、《突發(fā)公共衛(wèi)生事件應急條例》、《國家突發(fā)公共衛(wèi)生事件應急預案》。 1.4疫情分級 Ⅰ級:本省范圍內2地市及以上發(fā)現(xiàn)本地新型冠狀病毒的實驗室確診病例,并在人群出現(xiàn)新型冠狀病毒疫情擴散,屬特別重大疫情。 Ⅱ級:本省范圍內發(fā)現(xiàn)輸入性確診病例5例以上;或本地發(fā)現(xiàn)新型冠狀病毒的實驗室確診病例,并出現(xiàn)新型冠狀病毒疫情在人群擴散的趨勢,屬重大疫情。 Ⅲ級:本省范圍內發(fā)現(xiàn)輸入性新型冠狀病毒確診病例,且未出現(xiàn)本地感染病例;屬較大疫情。 Ⅳ級:本省范圍內發(fā)現(xiàn)疑似新型冠狀病毒病例疫情,屬一般疫情。 1.5適用范圍 本預案適用于我省各級衛(wèi)生行政部門及各級各類醫(yī)療衛(wèi)生機構開展的新型冠狀病毒防治應對準備及應急處置工作。 2組織管理 2.1組織機構 各級衛(wèi)生行政部門在本級政府統(tǒng)一領導下,成立新型冠狀病毒防控工作領導小組,統(tǒng)一指揮、協(xié)調新型冠狀病毒防控工作。 各級各類醫(yī)療衛(wèi)生機構實行新型冠狀病毒防控工作主要領導負責制、防控工作責任制和責任追究制,明確任務、目標和責任。 縣級及以上衛(wèi)生行政部門成立由臨床、流行病學、實驗室檢驗、衛(wèi)生監(jiān)督與院內感染等相關專業(yè)人員組成的新型冠狀病毒防控技術專家組。 縣級及以上醫(yī)療衛(wèi)生機構成立新型冠狀病毒疫情應急處置小組,根據(jù)職責分工和衛(wèi)生行政部門指派,負責開展本單位或本地區(qū)的新型冠狀病毒疫情應急處置工作。 農村鄉(xiāng)鎮(zhèn)(村)和城市社區(qū)衛(wèi)生機構在上級疾控機構和醫(yī)療機構的指導下,開展本地區(qū)的新型冠狀病毒防控工作。 2.2職責分工 2.2.1衛(wèi)生行政部門職責 (1)各級衛(wèi)生行政部門在當?shù)卣I導下,負責協(xié)調、管理本轄區(qū)新型冠狀病毒防控工作,制定本地新型冠狀病毒應急預案和防控策略,進行社會動員并指導本轄區(qū)內做好新型冠狀病毒防控工作;組織開展新型冠狀病毒專業(yè)人員培訓和應急演練;確定定點收治醫(yī)院;組建專家組診斷、指導治療本轄區(qū)內新型冠狀病毒病例,為防治工作提供技術支持;開展防控工作的督導檢查;做好應急防控物資儲備;加強與相關部門的協(xié)調與配合,建立部門之間信息溝通和固定聯(lián)絡員制度,形成多部門共同參與的聯(lián)防聯(lián)控機制。 2.2.2醫(yī)療衛(wèi)生機構職責 (1)疾病預防控制中心職責 ①省級疾病預防控制中心負責制訂本省疫情應急處置技術方案;評估和預測本省疫情;參與并指導現(xiàn)場流行病學調查及疫情處置;指導新型冠狀病毒預防控制工作;負責新型冠狀病毒疫情及監(jiān)測資料的收集、匯總分析、反饋和上報;開展技術培訓和健康教育;開展實驗室檢測工作,并保障實驗室生物安全,負責業(yè)務人員的培訓。 ②地市級疾病預防控制機構負責本地(市)新型冠狀病毒疫情及監(jiān)測資料的收集、匯總分析、反饋和上報,指導地市、縣市區(qū)新型冠狀病毒預防控制和監(jiān)測工作,開展現(xiàn)場流行病學調查及疫情處置,做好技術培訓和健康教育,在條件具備時開展實驗室檢測工作并保障實驗室生物安全,負責縣(市、區(qū))業(yè)務人員的培訓。 ③縣、市(區(qū))疾病預防控制機構承擔本轄區(qū)新型冠狀病毒預防控制及監(jiān)測工作,負責當?shù)匾咔榧氨O(jiān)測資料的收集、匯總分析、上報,開展新型冠狀病毒病例的流行病學調查,密切接觸者追蹤和醫(yī)學觀察,相關標本的采集和運送,指導做好消毒工作和新型冠狀病毒疫情現(xiàn)場處置人員的個人防護,開展專業(yè)人員培訓和健康教育,負責鄉(xiāng)村衛(wèi)生人員的培訓。 (2)醫(yī)療機構職責 縣級及以上醫(yī)療機構負責新型冠狀病毒醫(yī)學觀察病例、疑似病例的篩查與報告,負責病人的診斷、轉運、隔離治療、醫(yī)院內感染控制,配合疾病預防控制機構開展流行病學調查及標本采集工作,負責本機構內有關人員的培訓工作。 鄉(xiāng)鎮(zhèn)(村)和社區(qū)衛(wèi)生機構以及其它各類醫(yī)療機構發(fā)現(xiàn)發(fā)熱、咳嗽、呼吸急促和呼吸困難等急性重癥呼吸道感染癥狀表現(xiàn)的病例,在上級部門的指導下開展新型冠狀病毒防控工作。 (3)衛(wèi)生監(jiān)督機構職責 負責對本轄區(qū)醫(yī)療衛(wèi)生機構的預檢分診、消毒、疫情報告及預防控制等工作的衛(wèi)生監(jiān)督和執(zhí)法稽查。 3病例診斷和疫情發(fā)布 3.1病例診斷 省內首例新型冠狀病毒病例由衛(wèi)生部專家組診斷確認,此后發(fā)生的病例由省級衛(wèi)生行政部門組織專家組診斷確認,同時上報衛(wèi)生部。 省級衛(wèi)生行政部門在接到轄區(qū)內新型冠狀病毒疑似病例報告后,應在2小時內派出專家組進行調查和會診,并向衛(wèi)生部報告。 省專家組根據(jù)病例的流行病學史、臨床表現(xiàn)、實驗室檢查結果,按照《新型冠狀病毒感染病例診療方案》(2012年第1版)進行診斷或排除。 省疾病控制中心開展新型冠狀病毒病例實驗室檢測工作;各地(市)級疾控中心在具備條件想情況下開展本轄區(qū)的實驗室檢測工作,需要復核時送省疾控中心檢測。 省疾病控制中心檢測出的所有陽性標本全部送中國疾病預防控制中心復核檢測。 3.2疫情公布與通報 省級衛(wèi)生行政部門經(jīng)衛(wèi)生部授權后,負責向社會發(fā)布本省新型冠狀病毒疫情信息。 4應急處置 各地應根據(jù)以下不同情況采取相應的應對措施。 4.1本省尚未發(fā)現(xiàn)疑似或確診新型冠狀病毒病例疫情,應該采取以下措施: (1)密切關注國內外新型冠狀病毒疫情動態(tài),做好疫情預測預警,開展疫情風險評估。 (2)做好各項技術及物資準備。 (3)開展常規(guī)疫情、新型冠狀病毒、不明原因肺炎病例的監(jiān)測。 (4)醫(yī)療機構開展不明原因肺炎的篩查工作。 (5)開展新型冠狀病毒知識的健康教育,提高公眾防控新型冠狀病毒知識水平。 4.2本地發(fā)現(xiàn)疑似新型冠狀病毒病例疫情,采取以下措施: (1)立即開展現(xiàn)場流行病學調查、密切接觸者追蹤和樣品采集工作。 (2)開展新型冠狀病毒應急監(jiān)測工作,發(fā)現(xiàn)新型冠狀病毒病例的地市實行疫情日/零報告制度。 (3)做好密切接觸者的醫(yī)學觀察。 (4)按照職責分工,做好疫點內人居住和聚集場所的消毒處理工作。 (5)醫(yī)療機構要做好病人接診、救治、醫(yī)院內感染控制等工作。 (6)做好疫情調查處理等人員的個人防護。 (7)各級衛(wèi)生行政部門均設立統(tǒng)一的12320咨詢熱線電話或專線電話,24小時解答群眾有關流感防治的咨詢、舉報和投訴。 4.3本省范圍內發(fā)現(xiàn)輸入性新型冠狀病毒確診病例,且未出現(xiàn)本地感染病例,采取以下措施: (1)加強新型冠狀病毒應急監(jiān)測,全省實行新型冠狀病毒病例零報告制度。 (2)按照中國疾控中心制定下發(fā)的《新型冠狀病毒預防控制技術指南》迅速開展流行病學調查工作,查明病例之間的相互關聯(lián)。 (3)按照密切接觸者判定標準和處理原則,確定密切接觸者,并做好醫(yī)學觀察。 (4)按照職責分工,做好疫點內人居住和聚集場所的消毒處理工作。 (5)醫(yī)療機構要做好新型冠狀病毒病例隔離、救治和醫(yī)院內感染控制工作,并協(xié)助疾病預防控制機構開展流行病學調查和病例的主動搜索、標本采集等工作。 (6)加強疫情調查處理、醫(yī)療救治、實驗室檢測等醫(yī)務人員的個人防護。 (7)及時向本地區(qū)有關部門和鄰近省(區(qū)、市)衛(wèi)生行政部門通報有關情況。 (8)進一步加強健康教育,提高公眾衛(wèi)生意識和個人防護意識,減少發(fā)生新型冠狀病毒的危險性,做好公眾心理疏導工作,避免出現(xiàn)社會恐慌。 4.4本省范圍內發(fā)現(xiàn)輸入性確診病例5例以上;或本地發(fā)現(xiàn)新型冠狀病毒的實驗室確診病例,并出現(xiàn)新型冠狀病毒疫情在人群擴散的趨勢,由疫情發(fā)生地地(市)新型冠狀病毒防治工作領導小組統(tǒng)一組織,開展防控工作。結合疫情發(fā)展趨勢,加強學校、農村、建筑工地、車站及公共場所等重點部位、重點場所、重點人群的監(jiān)測和預防控制工作。取消婚喪嫁娶宴請活動,禁止集市、廟會等群體性聚集活動。學校落實師生、員工晨檢制度,加強防治知識宣傳,保持校舍良好通風,定期進行消毒。必要時經(jīng)當?shù)匦滦凸跔畈《痉乐喂ぷ黝I導小組同意,可采取停課措施。影劇院、歌舞廳、網(wǎng)吧等娛樂場所要堅持嚴格消毒制度,必要時責令停業(yè)。交通、鐵路、民航實行健康申報和檢疫制度,發(fā)現(xiàn)疑似病例和密切接觸者時,采取就近就地隔離觀察、治療。相鄰市(地)要及時溝通信息,嚴密監(jiān)測。省有關部門應給予人力、物力、財力等方面的支持。 4.5本省范圍內2市及以上發(fā)現(xiàn)本地新型冠狀病毒的實驗室確診病例,并出現(xiàn)疫情在人群擴散,采取以下措施: 各地衛(wèi)生行政部門根據(jù)疫情流行情況,向當?shù)卣岢鼋ㄗh實施疫區(qū)封鎖、交通檢疫、停產(chǎn)、停業(yè)、停課等措施。 5保障措施 5.1加強技術培訓,提高應對能力 各級衛(wèi)生行政部門要加強對疾病預防控制人員的技術培訓,提高流行病學調查、監(jiān)測、消毒處理和實驗室檢驗的能力;加強對醫(yī)務人員新型冠狀病毒防治知識的培訓,要求所有醫(yī)療衛(wèi)生人員都要掌握新型冠狀病毒診療、預防控制和流行病學調查的相關知識,提高基層醫(yī)務人員早期發(fā)現(xiàn)病人的意識、能力和診療水平。 5.2完善檢測網(wǎng)絡,提高檢測能力 省疾病控制中心建立省級新型冠狀病毒檢測實驗室,各地(市)疾病預防控制中心建立新型冠狀病毒檢測實驗室。新型冠狀病毒檢測實驗室應符合實驗室生物安全有關規(guī)定和要求,配備專人負責,并選擇責任心強、技術水平高的專業(yè)技術人員承擔檢測工作。 5.3加強生物安全管理,確保實驗室生物安全 5.3.1各級疾病預防控制機構及科研機構要完善有關生物安全規(guī)章制度,配備必要的人員,健全實驗室安全管理制度,使生物安全管理做到科學化、規(guī)范化、制度化。 5.3.2開展新型冠狀病毒病毒檢測工作的實驗室必須符合我國實驗室生物安全的有關規(guī)定和要求,依照《病原微生物實驗室生物安全管理條例》、《可感染人類的高致病性病原微生物菌(毒)種或樣本運輸管理規(guī)定》、《微生物和生物醫(yī)學實驗室生物安全通用準則(WS233-2002)》和《實驗室生物安全通用要求(GB19489-2004)》等規(guī)定開展工作。在應急狀態(tài)下,省衛(wèi)生廳臨時指定合格的實驗室開展相關檢測。
